The unusual severed hand is a quest item in the Blood and Wine expansion for The Witcher 3: Wild Hunt.

The unusual severed hand is first encountered in the wine cellar of Corvo Bianco during Geralt's investigation into The Beast of Beauclair. Upon closer inspection, Geralt determines that the hand still has a heartbeat as though it were still part of a living organism.
Later, after pursuing the Beast of Beauclair into a lakeside warehouse, Geralt finds out that the hand indeed belonged to the Beast, who no longer requires it having grown a new one.
