This trap may quite possibly not have been implemented in the game. It is the same graphic, however, as the "bomb" Geralt hurls at the kayran at the end of the boss fight in Chapter I.
No traps are used for nekkers. Bombs used to destroy their nests, and those are grapeshot-type bombs.
