This potion can be concocted once Geralt obtains the formula from Triss. It can be made with ostmurk or any other alchemy ingredients that contain fulgur such as Endrega embryo. Despite what is said in the Journal entry, Mongoose offers excellent protection against any poison. It is very useful when fighting the queen endrega, for instance.
