Ass ears is a mask in the Hearts of Stone expansion.
They can only be received if one loses against the halflings playing gwent at the wedding reception, during the main quest, Dead Man's Party. Note: you'll also lose 5% of your money as well for losing.
Geralt can wear the ass ears by equipping them to a "pocket" in his inventory. If one also acquires The Professor's spectacles during Open Sesame!, the two can be combined using a diagram to get the concealment kit.
