Common graytop brew is a quest item in the Blood and Wine expansion.
It is very strong hallucinogenic made out of graytop (a nearly extinct mushroom) that Geralt can take if he wishes to help Pinastri during the secondary quest Equine Phantoms. When taken, it causes Geralt's vision to take on a mirage-like effect with a purple tint. The biggest benefit is that it allows Geralt to see umbras, which are invisible even to witchers under normal conditions.
There is no way to take the brew outside of the quest, and even if obtained through console commands it will not be consumable then. As Geralt receives and consumes it during a cutscene, it also can't normally be seen in the player's inventory.

- Pinastri has evidently been generous with the small amount of graytop that she had. In an ambient conversation overheard at the small shed east of Dun Tynne Castle, an old woman says that she got a present from a druidess (i.e. Pinastri) - powdered mushrooms that when brewed make you "see things you'd never see otherwise."
