If Geralt chooses Iorveth's path at the end of Chapter I: This item can be found, along with Baltimore's notes in a chest during the quest Baltimore's Nightmare.
If Geralt chooses Roche's path at the end of Chapter I: This item can be acquired by talking to Zyvik about the Battle of Brenna.
If Geralt attempts to deceive the wraith during the quest "Death Symbolized" while carrying this cap, the wraith will grant Geralt a second attempt at a question. Geralt is also granted an additional attempt if he has posession of the Dun Banner Cloak.
